Behavior contract--I would change the "don'ts" and reverse it so this was in positive language and boxes are not checked if a child misbehaves.

My Choices Today.note home to parents Make the student have accountability for their actions! I would add positive choices on the same list and if on a behavior plan have them check what they did do to reinforce the positive behavior choices.
