What's in a Name? Good as BTS Literacy Unit. Includes research, writing, reporting, technology use and bulletin board. For upper grade students I would also use  the book "House on Mango Street" by Sandra Cisneros. The chapter "My Name" would be great for discussion to share how students feel about their names.
