This daily behavior report card correlates with a clip chart in my store (see below). It is an easy way to hold students accountable and keep parents informed on their child's daily behavior during the school day. There is also a section to note what special events they attended that day, how they ate during lunch, and a place for other special information such as things they may have learned that day.

Use this practical chart on a daily basis and send it home to report back to your students' parents in a timely fashion. Circle one of the faces beneath each weekday so even the youngest students understand how their behavior has been evaluated. Then check off the appropriate description boxes and add any additional notes in the provided section {optional}. Enjoy this wonderful school-to-home communication tool. Can also be used at home and/or for homeschool!
